    MIDDLE COLLEGE GRADUATES ONLY!!!
     
    If you have graduated from another high school, please contact them directly to request a transcript.
    If you need a copy of your college transcript, please contact Contra Costa College.
     
    Please allow 24-48 hours for your request to be processed once received.
    Email requests will result in a faster response time.
     
    You may request a transcript by completing the MIDDLE COLLEGE HIGH SCHOOL TRANSCRIPT REQUEST FORM and submitting it to the main office. 
     
    Option 1 (August through June only) - Drop off completed form (in person or via mail):

          Scan and attach the completed MIDDLE COLLEGE HIGH SCHOOL TRANSCRIPT REQUEST FORM.
               If you are unable to scan the form, please include the following information in your email request:
                  - Legal Name while enrolled
                  - Date of Birth
                  - Year of Graduation
                  - Indicate the number of transcripts you are requesting
                  - Indicate if you are requesting an Official (signed, stamped, sealed) or Unofficial transcript
                  - Indicate if you are requesting it to be mailed or emailed
                  - Complete address of where you would like the transcript sent or emailed
                  - Contact information if we need further verification or have a problem processing your request
