Good Evening Ford Families,

We hope that everyone had a great weekend! Here are our reminders for this week:

1. The PTA meets this Thursday at 3:00 PM. We hope to see you there!

2. COVID testing at Ford is every Wednesday. We need families to sign up online or with the paper forms.

Online Forms:

The company would prefer that you sign up online. If you sign up online, you need to sign up for a testing appointment every week.

Paper Forms:
We have the paper forms in the office. If you completed a paper form, we also need either a copy of your insurance card, or a verification form filled out if you do not have insurance.
Copy of Insurance Card: Please email a picture of the front and back of your card to ford@wccusd.net with your child’s name and teacher, or stop by the office and we’ll make you a copy.

Uninsured Verification Form: They ask for a social security number, a driver's license lumber, or a state ID. If you don’t have one of these or would prefer not to share, that is totally fine. Please leave it blank. You can stop by the office to get this form. We also sent them home last week.

LSA will not test students this week without:
An online appointment
OR
A paper form with a copy of the insurance card or the Uninsured Form

Please help us keep our community safe by ensuring that your child gets tested every week!

3. Thank you everyone for helping make our school a safe place to learn and succeed. We would like to remind you that as part of the dress policy students must be wearing shoes that are safe for them to run and play in. Please make sure that your student is wearing closed toed shoes.

La empresa preferiría que se registre en línea. Si se inscribe en línea, debe inscribirse para una cita de prueba todas las semanas.

Formularios de papel:

Tenemos los formularios en papel en la oficina de Ford y los enviaremos nuevamente el lunes. Si completó un formulario en papel, también necesitamos una copia de su tarjeta de seguro o este formulario de verificación completado si no tiene seguro:

Copia de la tarjeta de seguro: envíe por correo electrónico una foto del anverso y reverso de su tarjeta a ford@wccusd.net con el nombre y el maestro de su hijo, o pase por la oficina y le haremos una copia.

Si no tiene seguro, simplemente complete este formulario. Se lo enviaremos a casa a todos los estudiantes mañana. Piden un número de seguro social, una licencia de conducir de madera o una identificación estatal. Si no tiene uno de estos o prefiere no compartirlo, está bien. Déjelo en blanco.

¡Ayúdenos a mantener nuestra comunidad segura asegurándose de que su hijo se haga la prueba cada semana!

Gracias a todos por ayudar a hacer de nuestra escuela un lugar seguro para aprender y tener éxito. Nos gustaría recordarle que, como parte de la política de vestimenta, los estudiantes deben usar zapatos que sean seguros para que puedan correr y jugar. Por favor, asegúrese de que su estudiante use zapatos cerrados.
