COVID Testing Forms

Hello families,
We have a new COVID testing company and we need to fill out new forms.
1. We strongly recommend doing this online. Please see the attached sheets for the instructions.
English Instruction Sheet

2. If you cannot do this online, please fill out the paper form that we sent home today.
Students will test with their class at Ford on Wednesday. The system will have you sign them up for an appointment, but the appointment is just a placeholder. Students will test with their class during their class testing time.

Tenemos una nueva compania para pruebas de COVID y necesitaran completar nuevos formularios.
1. Recomendamos que completen el formulario en línea. Consulte las hojas adjuntas para obtener las instrucciones.

Los estudiantes se les haran los analises con su clase en Ford cada miércoles. El sistema le pedirá que los registre para una cita, pero realizarán la prueba con su clase durante el tiempo de prueba de la clase.
