Predicine has been contracted by the district to oversees the Covid testing for all 38 elementary schools for WCCUSD.

Prior to having your child tested, please fill out the information on

• the Testing Request Form (Formulario de solicitud de prueba)
  • the "Patient Information" in the yellow box
  • AND the "patient/parent/guardian signature on the first page
    
  • AND the signature on the second page

Under insurance information, you may select "Test Location."

For "Phone" provide a number that receives text messages if you wish to receive your results via text.

• the Parental Consent Form (Formulario de consentimiento de los padres)

We understand there may be some students who have medical or religious obstacles in being tested. For those families, please follow these steps:

• Submit, in writing, a note to your school Principal that your family has a religious objection to testing
• Or a note from your doctor/provider stating your student has a medical condition where testing on campus would not be appropriate.
