Please read the following information on facility use carefully. You may print the form for your convenience or you may obtain a form from the district office:

Please fill in the Facility Request Form completely before submitting. Return the form to the above address, or fax it to (510) 620-2025. The Facility Use Office must receive the form (10) ten days prior to proposed date(s) of use. You may also fax the completed form to (510) 620-2025.
The West Contra Costa Unified School District requests a Certificate of Insurance and a copy of the Policy Endorsement from your insurance carrier naming WCCUSD as additionally insured and certificate holder. WCCUSD name and address should appear on the certificate and the endorsement. Insurance Limits are as follows:
